Performance Citation Award
Do you know a person or a group in N.B who is making an outstanding contribution in the healthcare field that reflects the missions of the CHANB?
If so, we invite you to nominate them to the Performance Citation Award, N.B.’s highest distinction in Catholic healthcare.
The Performance Citation Award winner will receive:

Recognition at the CHANB annual convention;

A distinctive personalized and professionally framed certificate;

Special coverage with photos in CHANB’s magazine: The Pulse;

An honorary life membership in CHANB
In addition, a media advisory with photos will be sent across the province to publicize the recipient’s accomplishments.
Selection criteria
The Citation Award winner:

Exemplifies leadership in an activity program or project worth of national recognition for it’s building up of the Christian community in an extraordinary way;

Shows unselfish dedication to others;
-
Makes a particular contribution to healthcare in a Christian context;
-
Is recognized by peers for this outstanding work.
Nomination Process
Any member of the CHANB or anyone working for a CHANB member may nominate a person or group whom they feel deserves to be recognized.
Complete the nomination form below and return it before July 31st.
The Performance Citation Award Committee will contact you to obtain more information on your nominee.
Eligibility
Any individual or group that has demonstrated, in a tangible way, a commitment to the values of the CHANB through their lifework or through a program which is a model for others or has a Provincial impact is eligible. Current members of the board of directors and staff of the CHANB are not eligible.
Judging
The Performance Citation Award Committee will choose the winner who will then be notified by the CHANB.
